Thelma Hall Chinault, 62, passed away from complications of ALS on Dec. 12, 2013.

Thelma was born in Daytona Beach, Fla., and lived in Polk County for the past 35 years. She owned and operated Hall Insurance Services in Bartow, Fla., for 28 years.

She is survived by her husband of 41 years, Gregory Chinault; son, Mathew Chinault (Katherine); two daughters, Lauri Shapiro (Matthew), Rebecca Duvall (Zeb); one grandchild, Whitney Shapiro; five siblings, Garey Hall, Viola Collins, Jerry Hall, Shirley Lucas, Kenneth Hall; and a large extended family.

In lieu of flowers, the family requests that donations be made to the ALS Association, www.alsa.org/donate/ or the Florida SPCA, www.spcaflorida.org
